codec/cc: Fix garbled DVD CEA-608 captions with large cc_counts

The following two patches are needed to properly render CEA-608 closed captions on DVDs (i.e. MPEG2 user data GOP encoded captions).

Issue reported on a Youtube video, "Closed captions on DVDs are getting left behind" on the "Technology Connections" channel.
