Purge very old obsolete options (from before v2.2 final)

Lyndon Brown requested to merge jnqnfe/vlc:obs_opts2 into master

(Commits renamed to avoid the confusion that occurred in the ML).

These commits remove those made obsolete only up to and including development of v2.2, exactly as suggested would be fine by Thomas in !88 (closed).

  • Purges options made obsolete before v2.0 development started
  • Purges options made obsolete during v2.0 development
  • Purges options made obsolete during v2.1 development
  • Purges options made obsolete during v2.2 development

All options were carefully researched at the time I made these as I recall, to double-check their history and check no code existed that related to them. Only one option was skipped due to that (--server-port, which will be dealt with separately) problem addressed by !124 (merged).
