macosx: Playback -> Program, Chapter aren't updated properly.

There are three type of files:

• A.mp4 has chapters (PartA, PartB) and doesn't have program.
• B.ts has programs (1000, 1002) and doesn't have chapters.
• C.mkv has chapters (Opening, Ending) and doesn't have program.

Playing A.mp4, B.ts, C.mkv and A.mp4 successively, the issue appears.

When, playing A.mp4, Program has no item, and Chapter has "PartA" and "PartB".

When, playing B.ts, Program has "1000" and "1002", and Chapter has "PartA" and "PartB".

When playing C.mkv, Program has "1000" and "1002", and Chapter has "Opening" and "Ending".

When playing A.mp4, Program has "1000" and "1002". and Chapter has "PartA" and "PartB".
