decoder: first Closed Captions blocks are always skipped

The CC description is sent from the decoder thread with a first block.

The input thread can now create a CC decoder when it is sending new data to the master decoder.

The newly created CC decoder can now receive blocks from the master decoder, but it may have missed the first few blocks.
