Fullscreen controls got lost (disappeared)

The only way FSC won't get lost is to use only context menu or "Toggle the video in fullscreen" button on the controller to enter / leave fullscreen.

Using OpenGL vout module causes the controller to show up even when not in fullscreen.

This happens after toggling FS many times.

Also there's double entry on vout module, direct3d vout in preferences.

FSC works fine in no-embedded vout except #2428 (closed)

This one doesn't happen anymore here. (not sure when it is fixed)

I think might be fixed by Qt 4.5.0.
