qt: redesign preferences interface 

 - sidebar introduced
    - replaces the simple/full/expert selection control.
    - the reset-all button has moved into it.
    - the hotkey editor and media library manager interfaces have been
      moved to it from the simple interface.
 - minor layout/spacing tweaks and such
