Create proper MP4 AVC files

VLC doesn't create proper MP4 files for AVC/H264 content. (They don't playback with QTP). The reason is that we don't write a AVC box with AVC private data.

The cause of this problem seems to be in ConvertAVC1() of mux/mp4.c or in PacketizerAVC1() of packetizer/h264.c
