Path to SD-card changed - does not load the playlists correctly

Description

Since an update it changed the mountingpoint of a SD-card. Instead of loding the files from /storage/<SD-card id> it loads it from /mnt/media_rw/<SD-card id>. Because this path doesn't exist anymore, it wouldn't load the playlists which were linked to the SD-card.

Is there a way to change the path without having to start over?

Behavior

Doesn't load the playlists connected to the SD-card.

Steps to reproduce

1. Open "More" in the navigation
2. Tap on "Settings"
3. Go to "Media libary folders"

There are now multiple SD-cards with the different mounting points.
