Add to Playlist dialog : multiple issues

Description

With a large number of playlists, the Add to Playlist dialog have 2 issues :

• dialog is centered in the middle of the playlists, so the dialog title is not visible
• the "Add to new playlist" should be at the top, because you have to scroll all your playlist if you want to use it.
