Fix #2

The :5000 is now unrequired, and is not present in some runners list of
authorized images

CI: added debian, macos, win32 and win64 build jobs.

Breaks MinGW GCC-5.1.

 * configure.ac: remove include constructs because aclocal isn't clever
   enough to look for missing macros in these files.

  * configure.ac: hide autotools files in .auto/.

* libdca/parse.c: Change output normalisation factor from 3/2 to sqrt(2). Thanks to Alexander E. Patrakov for finding that this is the proper normalisation factor.
                  Fixed a bug where the output bias wasn't applied when downmixing wasn't being done.
