Index is masked to 0-31, but queue size is 31.  Size should be 32.

especially after stdio.h as it redfines off_t which
isn't defined until then.

mingw64 doesn't have SEEK_* defined in unistd.h
It seems that posix compliant systems are starting
to require their definition only in stdio.h

 - validate object ID
 - program should not change if object ID is invalid

stream types, formats, rates, aspect, and char codes.

Both looking up the current chapter and looking up the
position of a chapter were broken.

Added BD_MUTEX
 - renamed WIN32 pthread_* wrappers to bd_mutex_*
 - added pthread wrappers with error-checking and support for recursive locking

m2ts_demux: Emit complete PES packets immediately. This eliminates demuxing delays and the need to flush demuxer at the end of stream. - use packet length from PES header (this drops support for PES packets larger than 65541 bytes) - do not wait for next PUSI
