macosx: get rid of the infamous 'get' prefixes everywhere and finally respect Cocoa's naming scheme here

This improves readability a lot and prevent us from reinventing the wheel.

Missing some cases that I could not test.
Also ffmpeg/chroma.c is locked, so I can but leave it broken.
Fix is for the remaining modules is obvious and the issue is easy to detect.

macosx: added an option to automagically save the equalizer status and its settings (default enabled). Code cleanup.

    - removed <10.3 support in the preferences' code
    - added preliminary support for Obj-C 2.0 / Garbage Collection
        - needs to be enabled manually in intf.h for now and is restricted to future Darwin release
    - updated various copyright dates to approbate values
    - configured the source tree in the Xcode project for future Xcode releases
    - finally renamed the Xcode project to the correct bundle extension

* Fixes initialization when a preset is saved in the prefs or provided from command line. Could be cleaner, but it seems at least to work.

* equalizer.m: use preset_list_text instead of preset_list when creating the popup-entries to enable l10n of the presets
* MainMenu.nib: made the equaliser's popup a wider so it fits all l10ns

 + macosx/vout*: --macosx-vout and --macosx-opengl-effect
                 no longer exist

intf: Equalizer window in window menu
equalizer.m: presets should work now
equalizer.c, Modules.am, equalizer_presets.h: presets are stored in a separate file

*.nib: presets disabled on startup
equalizer.*: add property Id
             we prefer to change playlist variables instead of config
             coding style
