      The previous implementation would potentially read the "element" right
      after the last one in the subsequently used array (as priv->i_title
      denotes the number of titles present).
      These changes also include a check to make sure that var_GetInteger(
      p_input, "title" ) does not result in a negative value (as such can be
      used to read from outside the array, but on the other side).

      I just noticed that this patch results in a warning due to
      `input_thread_private_t::title` being *const-qualified*, and `p_title`
      lacking such qualifier.

      Subject: [PATCH] input/control: INPUT_GET_SEEKPOINTS: apply const to silence
      priv->title[n] results in a pointer-to-const, which means that the
      initialization of p_title discards the const qualifier; this addition
      fixes that warning while also making the implementation more correct.

      VLC has status reporting on the input state, but not on the output state.
      This makes it impossible to know if playback actually started completely from
      checking stream status states.
      For instance a failed audio output cannot be determined programatically. This
      patch adds an extra field per ES (in src/input/es_out.c) to determine the state
      of that ES (DISABLE, ENABLED, ERROR).
      If CreateDecoder() cannot instantiate an output, then it set p_dec->b_error = true.
      This is used to determine what the return value for es_out_GetEsState() should be.
      If p_dec->b_error is true, then it returns an ES_OUT_STATE_ES_ERROR to the caller.
      A function 'es_out_GetEsState()' is available to query an input for the state of
      the primary audio, video and SPU elementary streams.

      This follows the historical LibVLC semantic (and new --rate one), so
      bigger is faster. In the process, simplify a few code paths, especially
      those in LibVLC, but make many other code paths more complicated due to
      integer<->float conversion. Most of those paths could probably be
       * Fix RC "fastforward" command behaviour (it was slowing down instead)
       * Fix str_format_mera 'R' format string with negative rates and
         rates such that (rate % 1000 < 100).
