It will allow to removes vlc_object_find(_*) hacks.

It could be used to update the netsynch module or to lock the input
clock on the audio one.

Partially based on a patch by Jean-Paul Saman.

 It will allows to control the moment at which the meta are updated
to fix the delay introduced by timeshift and buffering.
 It will also simplify the way meta data are read by demuxers.

No functionnal changes.

 The input variable "program-scrambled" will contain the state for the current
program only. If at least one es is scrambled then the whole program is.
 The associated event is INPUT_EVENT_PROGRAM.
 If necessary for a better GUI support, we could change it to a list.

It fixes a lot of unlocked access.
It will allows a better clock algo.

A missing 'b' flag in fdopen.

It is useless to read the stream data when flushing.

It should fix win32 build.

It is done by using __attribute__((__packed__)) when available.
