we can now read the .idx file. TODO:
  - create a structure to store subinfo in (like WAVEFORMATEX and BITMAPINFO
  - allow for multiple tracks in the sub demux
  - open the .sub vobsub file, remove the mpeg crud and prepare a packet
    for spudec.
