+ macosx/vout*: --macosx-vout and --macosx-opengl-effect
                 no longer exist

  + vout.m contains the common code for the QT video output and the
    GL provider (window creation, events handling)
  + opengl.c: higher priority on OS X
 Things are a bit broken atm, any help to debug is welcome ;p
